Travel Grant Program

LCN is honored to support the journeys of heroes born with a laryngeal cleft by helping offset the financial impact. We offer grants of up to $500 for travel to access medical treatment for laryngeal cleft care.

2024 RECIPIENTS

Program Information

    • Patient must be traveling at least 100 miles from their home.

    • Patient must have a confirmed diagnosis of laryngeal cleft as documented by a rigid scope procedure.

    • Travel must be to access medical care for the patient’s laryngeal cleft.

    • Applicant must have a financial need.

    • Travel must be scheduled in the future, or has occurred within the last 30 days prior to the application date.

    • May be used to seek a second opinion.

    • U.S. applicants at this time.

    • Up to $500 per family each year, depending on funding availability.

    • Grant requests are accepted on a rolling basis.

    • Grants are dependent upon funding.

    • Covered expenses include travel costs such as airline/train tickets, gas, lodging, rideshare, etc. Food will not be reimbursed.

    • Documentation required for application:

      • Rigid scope procedure report that states the patient has a laryngeal cleft and the type.

    • Documentation required for reimbursement:

      • Travel receipts (in the applicant’s name).

      • Proof that the trip was made to receive medical care for the patient’s laryngeal cleft.

      • NOTE: Screenshots or copies of bank account transactions will not be accepted as valid receipts.

    • Travel reimbursements will be issued by check from LCN and mailed to the applicant’s address.

    • If awarded, applicant will be asked to provide a a one+ paragraph summary about how the travel grant helped them and a signed release. A photo is optional.

    • Applicant will also be asked to submit the patient’s full journey to share on the LCN website.
